Hawkeye Paving Corporation is being fined $4,000 in penalties for a deadly bulldozer accident.

On December 11, 2012, 56-year-old Thomas McLachlan was pinned under a bulldozer after it rolled over on him.

The accident happened on Elmore Avenue in Davenport, Iowa.

The investigation by the Iowa Occupational Safety and Health Administration states that Hawkeye Paving did not do regular inspections or ensure the heavy equipment operators used safety measures like seatbelts.

According to the company's website, McLaughlin had been the company's lead operator since 1987.
