Got a question about the next phase of the health care law? Unity Point Health-Trinity has set-up a special hotline offering help.

People who don't have insurance can call and set up an appointment with a certified application specialist to answer questions and help get enrolled in the health insurance exchanges under the Affordable Car Act.

Call (309) 779-HLTH (4584) and leave your name and number. Application counselors will call you back.

As the insurance exchange provision of the Affordable Care Act (ACA) takes effect, Trinity will begin accepting appointments for uninsured community members to meet with specially trained enrollment specialists.  Certified Application Counselors from Trinity will walk applicants through the ACA enrollment process step by step to help sign them up for the insurance exchange marketplace. There is no cost for an appointment.

Counselors will help explain the Affordable Care Act, the Health Insurance Marketplace, qualified health plans, eligibility, premium tax credits, cost-sharing reductions and penalties for non-enrollment. Individuals with income less than $45,960 or a family of four with income less than $94,200 a year will qualify for savings under the ACA. Varying assistance based off of income is also available.

Those making an appointment are asked to bring:

  • Social Security numbers or document numbers for legal immigrants
  • Employer and income information for household members applying for coverage (paystubs / W-2 forms / wage and tax statements)
  • Policy numbers for current health insurance plans for your household
  • Employee coverage tool sheet available at unitypoint.org/marketplace completed for every job-based plan that a household member is eligible for even if they are not currently enrolled.

To make an appointment with a Certified Application Counselor, call (309) 779-4584. Further information about the Affordable Care Act and the Health Insurance Marketplace is available at unitypoint.org/marketplace.
